Permanent Jewelry Liability Waiver and Release Form

1. Acknowledgement of Risk: I understand that permanent jewelry involves the custom fitting and welding of jewelry, which carries risks including burns, skin irritation, allergic reactions, discomfort, or damage to the jewelry. I voluntarily assume these risks.

2. Health Disclosure

I confirm that I am not allergic to metals used (gold, silver, alloys). Have no medical condition/pacemaker that would interfere with service. Will inform of any allergies or relevant conditions.

3. No Guarantees

I understand there are no guarantees regarding the durability or quality of the jewelry, as lifestyle and skin type may affect its performance.

4. Liability Release

I waive and release Loveley Co., its owner (Elizabeth Crawley), employees, and affiliates from any claims or liability for injuries, errors, or damage arising from the service, including negligence.

5. Indemnification

I agree to indemnify and hold harmless Loveley Co.for any claims brought by third parties related to this service.

6. Photography Release (Optional)

I consent to the use of photos for marketing purposes.

7. For Minors If the Client is under 18, a parent/guardian must sign below: Minor’s Name & Consenting Parent/Guardian Name
